Originals of forms to document information required by licensing agencies regarding medication administration and health care procedures are included with each Health Counseling Services (HCS) nursing consultation contract.

HCS has been designing health care forms for over 25 years. We understand the varied needs for health records in community settings. Our health care forms are easy to use, clear and concise. Used in conjunction with HCS manual, the forms offer caregivers a documentation system that provides the opportunity to create accurate and complete health care records.

As needed, HCS revises the forms, supplying contracted organizations with updated new originals.
